The Center’s Goals
What is an IDS? Integrated Delivery Systems seamlessly coordinate the full spectrum of care for all patients. The goal of an IDS is to provide high-quality care, efficiently use resources, and improve patient and population health, as well as measure these results. Barriers to care — such as higher rates of comorbidity, limited access to transportation services and lower health literacy — make it particularly difficult for vulnerable patients to effectively navigate the current health care system. Safety net hospital systems understand this unique population and, with innovative care coordination models, can improve the health of patients and lower costs associated with inefficient use of services.
The Action Collaborative aims to engage NAPH members ready to participate in demonstration projects to create accountable care organizations, coordinated care networks and other models that require greater clinical and financial integration. Participants receive help developing proposals and interacting with funders, as well as extra support when accepted into specific projects. The Action Collaborative also will serve as a national voice within the NAPH and policy communities.
NAPH Members Providers of Choice Research Brief(May 2011) This brief summarizes the findings from 10 years of data collected as part of the NAPH Annual Characteristics Survey. The study reveals past challenges for NAPH members including growing service demands, limited capacity, changing payer mix, and uncertain financing.
