What is an IDS?
Integrated Delivery Systems seamlessly coordinate the full spectrum of care for all patients. The goal of an IDS is to provide high-quality care, efficiently use resources, and improve patient and population health, as well as measure these results. Barriers to care — such as higher rates of comorbidity, limited access to transportation services and lower health literacy — make it particularly difficult for vulnerable patients to effectively navigate the current health care system. Safety net hospital systems understand this unique population and, with innovative care coordination models, can improve the health of patients and lower costs associated with inefficient use of services.
The Transformation Center is a component of Essential Hospitals Institute, formerly the National Public Health and Hospital Institute. The center puts research into practice and fosters innovative change through education and tools, leadership development, and federal and state demonstration opportunities.
The center helps hospitals deliver integrated, high-quality, affordable, and population-focused care. A key aspect of this work is the Essential Hospitals Engagement Network, a part of the federal Partnership for Patients initiative to reduce hospital-acquired conditions and preventable readmissions.
